We are currently accepting new patients / clients.

  • Wed: 8 am – 9 pm
  • Mon, Tue, Thurs and Fri: 5 pm – 9 pm
  • Sat: 7 am – 7 pm
  • Sun: 3 pm – 9 pm

We charge $260 for an initial visit

  • $150 for your follow up,
  • $100 for no show appointments.

How to prepare for your virtual visit

All visits are done using Doxyme video. The provider text/email the link to the patient prior to appointment. Patients less than 18 years of age need parental consent as per the state of Ohio law. We charge $100 dollars flat fee payable by patient not insurance for all appointments cancel less than 24 hours. Insurance does not cover appointment cancellation.

Submit Medical Records, Labs & Medications

Complete labs prior to appointment as needed. Request past medical record from your provider(s) and fax to CFF. Email list of all medications taken, name of prescribers, pharmacy, address, phone and fax number

Connect to Doxyme video

Use a computer or device with a camera & microphone. Click the link from your provider. Type in your name and click Check In. Allow browser to use webcam & microphone. Wait for your provider to start the call.
